APPLICATION FOR LEAVE OF ABSENCE
Appointive officials, permanent, temporary and casual employees of the LGU who renders forty-hours a week is entitled to a 15 days’ vacation leave and 15 days sick leave credits annually without limitation as to number of days of leave credits that may be accumulated. Personnel who wish to apply for vacation and sick leave of absence may do so provided availing of such shall be subject for recommendation of the head of office and approval of the Municipal Mayor.   Availability of Service: Monday to Friday (8:00 AM to 5:00 PM)

2. TERMINAL LEAVE PAY AND MONETIZATION OF LEAVE CREDITS
Officials and employees who have accumulated at least fifteen (15) days of vacation and sick leave credits or more may be allowed to monetize his/her leave credits provided that at least five (5) days shall be retained after monetization and provided further that the maximum days to be monetized is thirty (30) days given in a year. Monetization of sick leave credits may be allowed for valid reason subject to the discretion of the agency head and the availability funds.   Availability of Service: Monday to Friday (8:00 AM to 5:00 PM)
